According to police, one car was waiting to turn right at the intersection of Kangaloon Road and Burradoo Road when another car collided with the rear at about 3.45pm on Thursday afternoon.
The collision caused the turning car to collide with a four-wheel drive travelling in the opposite direction.
A man and woman, both aged 81, suffered abdominal pain, sternum pain and possible spinal pain.
The man was taken to Liverpool Hospital, while the woman was taken to Bowral and District Hospital.
Three people were assessed at the scene including a 40-year-old female who had sternum pain, an eight-year-old boy who suffered abdominal pain and a seven-year-old girl who assessed as a precaution.
Southern Highlands Police sergeant Geoff Cook said the crash was caused by a combination of factors.
"Wet weather and maybe some inattention on one of the driver's part," he said.
Mr Cook said while crashes on these roads weren't common, it was important for motorists to take care on the roads and especially in wet weather.
"It's not a notorious road but there has been a couple of prangs here," he said.
"This is just another road that people need to pay more attention to.
"Slow down a bit, keep your eyes on the road and drive to the conditions."
Police, paramedics, NSW Fire and Rescue crews, the State Emergency Service and a tow truck all attended the scene.
