Councillor Grahame Andrews was voted as deputy mayor.
Cr Gair was up against former deputy mayor Garry Turland who received two votes to Gair's four.
Councillors Larry Whipper and Ian Scandrett abstained from the vote.
Cr Gair said he was grateful to lead Wingecarribee Shire once more.
"I'm pleased, I'm thankful and I know the next few months might be challenging but I am standing for the right reasons," he said.
"I will endeavour to do my best for the community and the rate payers.
"There's a lot still to do in relation to capital works program and infrastructure budget that we have put in place.
"We've got to work together to get everything delivered and completed."
Councillor Grahame Andrews said he was humbled to be elected as deputy mayor.
"I am humbled by it and I appreciate the support," he said.
"I think it's terrific. I hope that I can carry out the duties.
"I am very pleased but I am more pleased of the return of mayor Duncan Gair.
"I'm delighted that he's back and delighted to serve with him."
Mayor Duncan Gair and deputy mayor Grahame Andrews will serve together until the next local government election in 12 months time.
