A Southern Highlands Police spokesman said that the road worker had been injured after being hit with an orange barrier at a section of road works.
He said a truck driver had pulled to the side of the road near the barrier to allow other vehicles to pass and hit the barrier which in turn hit the road worker.
The spokesman said the injured worker was assessed and stabilised at the site and transferred to Liverpool Hospital by Toll helicopter.
Police said the man was expected to stay in hospital for "a couple more days".
Investigations are continuing, but no-one has been charged.
