The aim of the program is to prevent admission/readmission by providing a package of home care services to facilitate safety and independence in the home.
SASH is targeted specifically at people who:
- are 18 to 64 years of age
- are not eligible for the National Disability Insurance Scheme (NDIS)
- do not want to access the NDIS
- have no limited or no informal supports
SASH offers a package of services lasting up to six weeks at a time including:
- help with household tasks such as cleaning, washing and shopping
- assistance with bathing/showering, dressing and maintaining personal hygiene
- in home respite to give carers a break
- transport to and from medical appointments
Referrals are made by outpatient or community health staff including nurses, allied health therapists, social workers and other relevant staff.
Once the referral has been accepted, a case manager will be allocated to each recipient to assist with identifying needs and arranging services.
During the six weeks, the case manager will make referrals for ongoing services after the SASH finishes or request another SASH package if applicable.
The SASH program is heavily subsidised by the NSW government so recipients are requested to only make a small contribution of $10 per week towards services.
