Moss Vale Motor Group, part of Wakeling Automotive, is donating $25 from the sale of every new and used car to the hospital.
'Wheels For Life' has given more than $1 million worth of equipment to the Campbelltown and Camden hospitals since 1997.
Now Bowral and District Hospital will benefit, starting with a Zoll defibrillator monitor valued at $10,000.
"We're very excited about this initiative," said the Bowral hospital's acting general manager, Valerie Jovanovic.
"Thanks for helping us provide the best care we can for our patients and equipment for our staff."
Wakeling Automotive chairman Paul Wakeling said group had long been "sponsoring this and that, but wanted to be more engaged with the community, and them with us."
As the money accrues, the hospital will identify equipment that would be "unprocurable from the health budget," Mr Wakeling said, "stuff that can be life-saving."
Wheels For Life is "a two-way street. The more the community supports us, the more money goes into the pot," he said.
South Western Sydney Local Health District chief executive Amanda Larkin said the initiative was sure to succeed.
The hospital had been in the community for "more than 125 years, and one of its rich things is its connections".
"It is strongly supported here. The richness of that relationship, the Wakelings understand."
Moss Vale Motor Group general manager Brett Wardle said that between the Moss Vale and Mittagong yards, "they sell about 100 cars a month".
"Now is the time to give back to the community," Mr Wardle said.
